We approached this the only way that seemed useful: rather than describe a marketplace everyone already knows, we searched it for the things a person on a GLP-1 medicine might plausibly buy, and reported what came back. On 8 August 2026, "insulin syringe" returned 5,293 results, and every listing we could read on the first page was captioned for animals or laboratories — "For Pet Farm Animal Cat Dog Pig", "for Animals, Lab", "Individually Packed for Feeding Pets", "for Pets Veterinary Use CE&ISO Certified". Not one presented itself as a device for injecting a human being. That is not an accident of phrasing. Syringes and hypodermic needles intended for human use in the United States are regulated medical devices with clearance and quality-system requirements covering sterility, needle geometry and barrel accuracy; labelling the same object for farm or laboratory use routes around all of it. Some of those syringes are probably fine. You have no way to tell which, no lot traceability you can act on, and no accountable domestic distributor if a batch is wrong — and the failure modes are dose error, tissue trauma and infection, weekly, for as long as you are on treatment.

A search for "sharps container" reporting 5,777 total results with listings from $0.99, predominantly captioned for tattoo, piercing and microblading studios. A search for "insulin cooler bag" reporting 5,341 total results with listed prices from $0.99 to about $9.32, including 'With Thermometer Display Insulin Cooler Travel Case Waterproof Thermal Insulated Refrigerated Medicine Organizer Bag' at $9.32 and 'Waterproof Insulin Cooling Bag Portable EVA Thermal Insulated Insulin Cooler with Gel Pen Bag Diabetics' showing 2,000+ sold at 4.9 stars. Searches for 'tirzepatide', 'GLP-1', 'peptide vial', 'pen needle', 'pen needle 32g' and 'bacteriostatic water' returned AliExpress CAPTCHA Verification pages on repeated attempts, including from a scripted real browser with fresh sessions and delays between requests; we therefore make no claim about what those searches return, and readers should not infer either presence or absence of such listings from this review. Regarding import duty: de minimis treatment was suspended for products of China and Hong Kong effective 2 May 2025 and for products of all other countries effective 29 August 2025, since when commercial goods entering the United States are subject to duty and to formal or informal customs entry regardless of value or shipping method; the One Big Beautiful Bill Act of 2025 repeals the underlying Section 321 de minimis provision for commercial shipments effective 1 July 2027. Regarding counterfeits: in December 2023 the FDA warned consumers not to use counterfeit Ozempic (semaglutide) injection 1mg found in the US drug supply chain, identifying lot number NAR0074 and serial number 430834149057 and stating that the needles were counterfeit and their sterility could not be confirmed, presenting an increased risk of infection; the World Health Organization issued a Medical Product Alert on falsified semaglutide following identification of batches in Brazil and the United Kingdom in October 2023 and the United States in December 2023; further counterfeit lots have subsequently been publicised, including instances using authentic lot numbers with counterfeit serial numbers. We attempted to retrieve the FDA alert page directly and received a 404, as FDA has reorganised these URLs; we therefore report the alert's substance as consistently documented across sources rather than quoting a live page, and readers should search fda.gov for the current version. The FDA declared the tirzepatide shortage resolved on 19 December 2024 and the semaglutide shortage resolved on 21 February 2025.
