What Semaglutide Actually Does
The trial evidence
In the STEP 1 trial, adults with overweight or obesity without diabetes taking semaglutide 2.4 mg weekly alongside lifestyle support lost a mean of 14.9% of body weight at 68 weeks, against 2.4% on placebo (Wilding et al., New England Journal of Medicine, 2021). Two things to hold onto: that took well over a year, and it studied the FDA-approved branded product. Compounded semaglutide has not been through equivalent trials, and shares an active ingredient rather than an evidence base.
Compounded semaglutide is not FDA-approved
Almost every price on this page is for a compounded preparation. Those are made by licensed compounding pharmacies for patient-specific prescriptions; they are not reviewed by the FDA for safety, effectiveness or quality, and they are not equivalent to Ozempic or Wegovy. Semaglutide came off the FDA drug shortage list in February 2025, and once a drug is no longer in shortage, pharmacies may not routinely compound what amounts to a copy of the commercial product — so ask any provider what clinical basis supports compounding in your case.
Lean mass is the thing to plan for
The STEP 1 body-composition substudy found roughly 45% of the total weight lost came from lean tissue. Resistance training — not aerobic exercise — is what the evidence supports as the countermeasure, alongside protein intake commonly targeted at 1.2–1.6 g per kg of body weight per day during active loss. That is harder to hit when your appetite has been suppressed, which is exactly why it needs planning rather than good intentions.
